We consider deuteron formation in heavy ion collisions at intermediate energies. The elementary reaction rates (NdNNN etc.) in this context are calculated using the Faddeev equation which is consistently modified to include the self energy corrections and Pauli blocking factors due to surrounding nucleons. We have calculated the life time of deuteron fluctuations and the chemical relaxation time in nuclear matter for densities and temperatures typical for the final stage of a heavy ion reaction. A microscopic approach to describe a heavy ion collision uses the Boltzmann equation that requires reaction rates in the collision integrals. A coupled Boltzmann equation for nucleons and deuterons is numerically solved using in-medium (NdNNN) reaction rates. We find that the relaxation time scales as well as the number of emitted deuterons predicted by the Boltzmann-Uehling-Uhlenbeck simulation change when medium dependent rates are used.
